Develop a Quote

Summaries of Benefits and Coverage Documents

To access the Summaries of Benefits and Coverage documents for quoting purposes, use the SBC Tool . If you need help, use the SBC Tool Guide .

Prepare a Quote

  1. Use the Mid-Market RFP Checklist to prepare your quote.
  2. Complete the Mid-Market RFP Form .
  3. Email the form to us.

Get New Sales Forms

Go to Form Finder. Click on Mid-Market Group from the left menu. You'll need to complete the following:

Benefit Program Application (BPA) for Mid-Market Groups
Use a BPA to provide employer information, check eligibility requirements and more.

Benefit Plan Selection Form (BPS) for Mid-Market Groups
Identify medical, dental and life plan selections. 

Employer Group Information (EGI) Form
Submit required documentation on group size, MSP and COBRA status.

Submit an Enrollment Request

Get the Mid-Market Enrollment Guide .

After logging in to your Blue Access for ProducersSM account, click Group > Enroll a Group > Mid-Market Group Enrollment. Instructions are on the e-Sales tools webpage.

For member enrollment, use Smart Census OR the Group Enrollment Application/Change Form.

Smart Census Import Tool

  1. After logging in to your Blue Access for ProducersSM account , click Group > Enroll a Group > Mid-Market Group Enrollment. Instructions are on the e-Sales tools webpage.
  2. Use the Quoting Census Template
  3. Follow the Smart Census Import Tool User Guide.

Group Enrollment Application/Change Form

  1. Go to Form Finder
  2. Search for the Group Enrollment Application/Change Form. Download the PDF for a new or existing member.
  3. The producer or employer can submit completed forms via the Membership Message Center. You need the "broker role" in your client's Blue Access for EmployersSM account, which you can access after logging in to your account  first.
