SWHR includes Texas Health Resources and UT Southwestern hospitals, other health care facilities, doctors and health care professionals. If we don’t sign new contracts, SWHR will leave these networks on April 1:
- ParPlan
- Blue Choice PPOSM
- Blue EssentialsSM
- Blue Advantage HMOSM
- Medicaid
- Blue Cross Medicare Advantage (PPO)SM
- Blue Cross Medicare Advantage (HMO)SM
SWHR doctors and hospitals will remain in the MyBlue HealthSM network.

If we can’t reach an agreement, or if you want to find another in-network doctor or hospital, we’ll work with you to move your care:
- Visit bcbstx.com and select Find Care to view our online provider directory, or visit your provider directory
- Register for or log in to BAM. If you don’t have access to BAM, go to the website on the back of your member ID card.
- Call the number on your member ID card
In the case of a life-threatening illness or injury, call 911 or go to the nearest emergency facility right away. Emergency services for commercial networks are covered at the in-network benefit level.
