Tips For Completing the Form
- Independent Producers: You'll need your National Producer Number (NPN).
- Agencies: Those working on behalf of a licensed Agency may complete the Agency form. You'll need your Agency's NPN and your Agency Principal's NPN.
- Subproducers: If you are a subproducer working for an agency, do not complete this form. Once an agency contracts with us, we work with the agency to "onboard" their subproducers.
