The Agency for Healthcare Research and Quality (AHRQ) Put Prevention Into Practice
Put Prevention Into Practice (PPIP) program presents A Step-by-Step Guide to Delivering Clinical Preventive Services: A Systems Approach. The Guide provides easy-to-follow, logical steps.  It’s designed for use by various audiences—physicians, nurses, health educators, and office staff—in public health clinics, community health centers, private practices, and other settings.

Physician Consortium for Performance Improvement
The American Medical Association (AMA)-convened Physician Consortium for Performance Improvement® (Consortium) is committed to enhancing quality of care and patient safety by taking the lead in the development, testing, and maintenance of evidence-based clinical performance measures and measurement resources for physicians. Provides measurement set descriptions and data collection flowsheets for 31 different clinical conditions.

Practice Improvement Modules (PIM)
PIMs are web-based self-evaluation tools that guide physicians through chart abstraction, patient survey and practice system inventory to establish practice performance assessment. The interactive report guides physician reflection on detailed performance data, areas for improvement, and creation of an improvement plan with goals and strategies. Once the plan has been tried and its effect measured, the diplomat reports the results to the American Board of Internal Medicine (ABIM). In addition to Maintenance of Certification credit, physicians who complete a PIM earn 20 CME credits.

METRIC- Measuring, Evaluating and Translating Research Into Care
METRIC is an innovative online practice improvement program that allows you to earn CME credit in your office, while improving patient care. The program is designed to assist family physicians in fulfilling the requirement for Part IV of Maintenance of Certification.

The Chronic Care Model
The Chronic Care Model draws on the best from research and practice, and provides a foundation for collaborative programs, tools and research aimed at improving care for the chronically ill. The Steps for Improvement Manual is an attempt to assist individual practice teams to begin improving their care for chronically ill patients.

The American Academy of Family Physicians
The American Academy of Family Physicians also has a comprehensive list of tools and resources in its Clinical Quality Improvement section. You’ll find information and resources to help you select a QI topic, implement a QI project in your practice, research clinical information systems and decision support options, stay updated on performance measurement and pay-for-performance, locate quality improvement activities in your state, and redesign your practice to provide optimal patient care.

The Medicare Quality Improvement Community
MedQIC is a national knowledge forum for healthcare and quality improvement professionals. It provides easy access to quality improvement resources and a community of professionals sharing knowledge and experiences to accelerate healthcare quality improvement across the nation.

Continuous Quality Improvement (CQI)
Continuous Quality Improvement (CQI) is a concept that came out of the business industry. Rather than creating a culture of blame if things do not go well, the focus is on a team approach to improvement that rewards the group when things get better. CQI has been adapted for health care in several ways.

Putting Quality into Practice (PQIP)
Putting Quality into Practice (PQIP), a joint effort of the ABIM Foundation and The Commonwealth Fund, is a research study designed to understand why physicians (especially physicians in small practices) have introduced quality measurement and improvement tools into their practices, and what has been the impact of doing so. The ABIM Foundation currently offers a DVD with physicians discussing quality improvement data for their practices and how they improved their professional and personal satisfaction.
