Provider Demographic Change

Q: What do I need to do if I am changing my tax ID?
A: Contact Provider Administration at 972-996-9610 and request a solo or group provider record application. You should request a group application if your practice is a corporation or a partnership.

Q: What do I need to do if I am changing my address and how do I add additional addresses to the provider directory?
A:

At least 30 days in advance of the change,

  1. Complete the "Change Your Information" form on the BCBSTX Provider Web site, or
  2. Contact Provider Administration at 972-996-9610, or
  3. Send written notification by mail or fax to your local Professional Provider Network office.
Q: My demographic information (specialty, practice information/status, board certification, etc.) is not correct on the Web site or I have added a sub-specialty. How do I correct this?
A:

At least 30 days in advance of the change,

  1. Complete the "Change Your Information" form on the BCBSTX Provider Web site, or
  2. Contact Provider Administration at 972-996-9610, or
  3. Send written notification by mail or fax to your local Professional Provider Network office.
