Contact Provider Administration at 972-996-9610 and request a solo or group provider record application. You should request a group application if your practice is a corporation or a partnership.

At least 30 days in advance of the change,
- Complete the "Change Your Information" form on the BCBSTX Provider Web site, or
- Contact Provider Administration at 972-996-9610, or
- Send written notification by mail or fax to your local Professional Provider Network office.
