PPO Select^® Choice Series III, a Preferred Provider Health Insurance Plan, emphasizes preventive care with coverage for routine physicals, well-child care, immunizations and diagnostic testing related to preventive services. When network providers are used for medical care, office visits and certain office setting services are generally covered in full, subject to the copay. So, depending on the health insurance plan policyholders choose, they could receive the services below for $25 per visit! Some services, such as immunizations for children through age seven (7), are covered at 100 percent of the allowable amount, no copayment required.
