Preferred Provider Health Insurance Plan (PPO) Select Choice Series III offers your clients many choices that help control the monthly premium and limit out-of-pocket health care costs, including the choice of deductibles and choice of health care providers. With eight (8) plan options to choose from, PPO Select Choice Series III is flexible enough to meet your clients' needs, family situation, and your budget.

Premiums are based on health insurance plan option, age, sex, geographic location, tobacco use, number of family members to be covered and evaluation of health risk. The higher the deductible chosen, the lower the premiums can be! Initial rates are guaranteed for six months except for entry into a premium age bracket due to a birthday or a change in residence that results in a different rate.
