	Select Blue Advantage Series III A preferred provider health benefit insurance plan that features a choice of deductibles and office visit copays, including same day lab and x-ray, a prescription drug card program and individual, spouse and child(ren) coverages available, including child-only coverage.
	PPO Select^® Choice Series III A preferred provider health benefit insurance plan that features a choice of deductibles and a low copay for in-network physician office visits.
	PPO Select^® Saver Series III A preferred provider health benefit insurance plan offering a back to the basics health insurance plan, designed to provide a selection of benefits, including a choice of deductibles, to fit your health care coverage needs and budget.
	BlueEdge^SM Individual HSA If you like to take charge of your health insurance and be responsible for how you spend your health care dollars then BlueEdge Individual Health Savings Account (HSA), a preferred provider health benefit insurance plan can give you the edge on health care!
	PPO Select Value^® Care^SM This preferred provider health benefit insurance plan offers you several health insurance options that help you control the monthly premium as well as your choice of health care providers from the BlueChoice network.
	PPO Select Basic^SM This preferred provider health benefit insurance plan, either in whole or in part, does not provide state-mandated health benefits normally required in accident and sickness insurance policies in Texas. This standard health benefit insurance plan may provide a more affordable health insurance policy for you although, at the same time, may provide fewer health benefits.
	Foundation Hospital Care Foundation Hospital Care, a preferred provider health benefit insurance plan, from Blue Cross and Blue Shield of Texas, offers a limited benefit "inpatient facility only" individual health insurance plan. This plan provides health insurance coverage for those situations where inpatient care is necessary.
