To help HealthSelect participants manage their health, BCBSTX offers a Health Risk Assessment. Once completed, you are provided with an online individualized report that gives you guidance and suggestions on the next steps to improving your health - including topics you may want to discuss with your physician. Helping real people manage their health.
The Health Risk Assessment is completed within the Personal Health Manager once you have logged in to Blue Access® for Members. The Health Risk Assessment is easy to understand and takes approximately 10 to 15 minutes to complete. As an added incentive, you will earn 5,000 Blue Points annually for completing the Health Risk Assessment (2,500 for completing one every six months). Taking the assessment every six months is a great way to track your progress in achieving better health.
If you are a current HealthSelect participant, you can complete your Health Risk Assessment by logging in to Blue Access for Members and follow these simple steps:
- Once logged in to Blue Access for Members, click on the Personal Health Manager logo found on the home page.
- Once in the Personal Health Manager, you will see a section called "Know Your Risk" located in the upper right of the screen. Within this section, click on the link to complete a Health Risk Assessment.
- Have your subscriber number and group number ready - you can find this on your HealthSelect ID Card.
- Go to Blue Access for Members, and click the Register Now button.
- Create a User Name and Password for immediate, secure access to your personal information.
If you are a future HealthSelect participant, you will have access to the Personal Health Manager and the opportunity to complete a Health Risk Assessment once your coverage goes into effect.
