Blue Care Connection is BCBSTX's wellness and disease management program that focuses on all participants. This unique approach provides support and guidance for those living with chronic conditions, such as asthma and diabetes, as well as those who are healthy or need help getting back on track to health and well-being.
Components of Blue Care Connection include:
- Personal Health Manager - the online tool that helps you manage your health
- Blue Points - earn rewards for healthy activity, which you can redeem for rewards
- Complete the Health Risk Assessment and earn 5,000 Blue Points (2,500 every six months)!
- Wellness Programs - reminders for important health screenings
- 24/7 Nurseline - experienced nurses available to you by telephone
- Special Beginnings® - mother and baby program
- Disease Management - caring advisors working with you and your doctor
- Lifestyle Management - tobacco cessation and weight-loss tools
