| BlueCare® Dental PPO |

BlueCare Dental PPO is a preferred provider option offering maximum
cost savings. Members avoid balance billing when they visit a BlueCare Dental PPO dentist or a DentaBlue dentist.
The BlueCare Dental PPO plan is available for self-insured groups and offers both standard and tailored plans. Members experience lower out-of-pocket costs when they choose a dentist in the BlueCare Dental PPO network. Members may also choose to use a DentaBlue dentist. When using a DentaBlue dentist, members will receive benefits based on the BCBSTX allowable amount and they will not be balance billed. In addition, they will not be responsible for charges over the allowable amount and may not have to file claims. Members will be responsible for any coinsurance or deductible as well as limited and non-covered services.

Benefits are also available for members who use a dentist who does not participate in the BlueCare Dental PPO network or in DentaBlue, however members will be responsible for charges over the allowable amount and may have to file claims.
The plan offers employees a variety of features for members using a network dentist, including:

Access to one of the largest dental PPO networks in Texas, with over 6,200 providers, and nearly 58,500 providers nationwide
Providers who do not participate in BlueCare Dental PPO or DentaBlue do not accept the Allowable Amount as payment in full. For services received from a non-participating provider, you will be responsible for any difference between the dentist’s charge and the Allowable Amount for the covered dental service.
